Initial Visits - Chiropractic Examination & Adjustment
($135): Dr. Whitham will take a thorough history and review of systems, followed by a comprehensive examination of the
areas of specific complaint; including functional muscle tests, central and peripheral nerve distress, local flexibility, joint integrity, cranial nerve status and vital functions. Dr. Whitham may
choose, based on his findings, to take radiographs (x-ray) to better assess the situation and ensure proper correction. Further evaluation, such as blood, saliva or urine laboratory testing, hair
mineral analysis or questionnaire completion may be requested before making final decisions. If Dr. Whitham is able to accept your case, a full report will be conveyed detailing the complete
findings, cause (if known) of discomfort, recommendations for care (including risks and any alternate options for treatment) with specific expectations for progress. An adjustment will be given on
the first visit, unless there are considerations that require further evaluation.
(NOTE: Appropriate & Necessary X-ray images included in initial exam at no cost.)
Follow-up images:
Cervical X-rays: $30/image
Thoracic X-rays: $45/image
Lumbar X-rays: $45/image
Chiropractic Adjustment ($50):
After a complete Functional Chiropractic Examination and current history, specific corrections are made, using a "test-retest
model" to restore proper alignment, balance and coordination to the nerve structures, joints and surrounding tissues (bones, muscles, tendons, fascia and ligaments). Patients find we are able to
accommodate all age ranges and body types with our wide array of technique options, including:
Diversified method - a traditional style of adjusting, done by hand, utilizing a thrust correction. This technique is requested or desirable in certain
situations and often produces the familiar "popping" sound, which is known to be gas escaping from a closed joint (and not a sound made by the bone itself).
Activator method - an advanced adjusting technique that
utilizes an Activator tool, also known by our younger patients as "the clicker." This technique is requested or desirable in sitautions where a more specific or reduced force is needed. This can be
valuable for young children or those with reductions in bone mass.
Myofascial release/Active release - involves a hands-on
release of soft tissue, such as muscles, fascia and tendons. Techniques, such as this, can be used to restore proper tissue length, tension and responsiveness. This is helpful in rehabilitation
post-injury or for chronic, unresponsive musculoskeletal conditions.
Applied Kineosiology - a method of identifying
underlying roots of injury throughout the body. Simple examples being a digestive problem that produces middle thoracic pain or a foot problem that causes lower back pain.
The Dr. will discuss these options with
you and obtain consent before making adjustments.

Initial Visit - Neuro-Emotional Technique(NET)/Stress &
Sensitivity Evaluation and Treatement ($145): Dr. Whitham or Dr. Norman will take a thorough history and review of
systems, followed by a comprehensive examination of the areas of specific complaint; including physical inspection, vital functions, and common substances for sensitivity. These may a limited
selection of the following: brain neurotransmitters, hormones, endocrine system factors (hypothalamus, pituitary, adrenal, thyroid and reproductive system) and immune regulators. Further evaluations,
such as basic and expanded food groups, vitamins, minerals, blood, saliva or urine laboratory testing, hair mineral analysis or questionnaire completion may be requested before making final decisions. If the
doctors are able to accept your case, a full report will be delivered detailing the complete findings, cause (if known) of discomfort, recommendations for care (including risks and any alternate
options for treatment) with specific expectations for progress. A treatment will be given on the first visit, unless
there are considerations that require further evaluation.
Neuro-Emotional Technique (N.E.T.) ($60):
Based on principles of Classical Conditioning, Nobel-Prize winning research in brain-body receptor chemistry and the
Chiropractic Adjustment, N.E.T. allows the body to release stored mental stress (limbic system) from the physiology through a simple, non-invasive process. After experiencing this procedure many
patients report feeling less stress, increased mental clarity, enhanced productivity, diminishment or elimination of physical symptoms (i.e. chronic pain, headaches, food and environmental
hypersensitivities, weight gain, organ dysfunction and chronic fatigue). Alive With Energy Clinic is one a few clinics in the Midwest with Physicians certified in N.E.T.

Initial Visit - Nutritional/Functional Medicine Evaluation ($85):
Dr. Whitham will take a thorough history and review of systems, followed by
a comprehensive examination of the areas of specific complaint; including physical inspection, vital functions, dietary analysis, metabolic status, and potential nutritional weaknesses. Further
evaluation, such as blood, saliva or urine laboratory testing, hair mineral analysis or questionnaire completion may be requested before making final decisions. If Dr. Whitham is able to accept your
case, a full report will be scheduled detailing the complete findings, cause (if known) of discomfort, dietary and nutritional medicine recommendations with specific expectations for
progress. ____________________________________________________________________________________________
Ancillary Services
Blood Lab Testing (Price Varies): Complete Blood Count, Metabolic Panel, Thyroid Panel, Lipid Panel, Hormone Panel,
Inflmmation Panel, Autoimmune Panel, nutrient values and other test are
available at Dr. Whitham’s recommendation through our local laboratory provider.
Brain Span ($125): A finger prick blood test and computerized test that measures several parameters of cognition,
memory and focus; omega-3/omega-6 ratios, Arachadonic Acid/EPA ratios and Palmitic acid levels -- all indicators of cell health and inflammatory burden. This comes with a 12-pages written report and
detailed supplemental and lifestyle recommendations. Learn more.
GI-MAP Stool Test ($400): A qPCR analysis of the gut/digestive microbiome. This test screens for levels of normal probiotic
conents, abnormal or overgrown bacteria, viruses, fungus, yeast, candida, parasites, potential autoimmune triggers, as well as the health of the intestinal wall ("leaky gut") and an invidual's
genetic compatibility with common antibiotic treatments. Learn more.
Saliva Hormone Testing (Price Varies): Saliva testing gives accurate, time-specific information regarding male, female, stress and sleep related hormones.
Urinary Neurotransmitter Screening (Price Varies):
Urinary neurotransmitters may be assessed in conjunction with a therapeutic neurotransmitter precursor program or to determine a
potential symptomatic cause.
Hair Mineral Analysis ($150 initial/$110
retest): A small, inconspicuous sample of hair is collected from the nape of the neck and analyzed by
our laboratory for standard mineral, trace mineral and heavy metal content. Patients are given a full report outlining their specific deficiencies and patterns, along with a thorough explanation of
mineral function, metabolic typing, nutritional recommendations and graphical representation of findings. Mineral deficiency or excess may indicate the need for nutritional support, mineral balancing
or biomedical detoxification. Learn more.
Injectable Nutrients (cost varies):
When certain indicators exist, Dr. Whitham may recommend intramuscular injections of specific nutrients and/or medicines to aid
the body’s recovery.
Biomedical Detoxification: The human body is subject to ever-increasing loads of dietary, chemical, environmental and emotional toxins. Over time, the accumulation of these
materials may lead to chronic fatigue, joint pain, digestive discomfort, decreased mental clarity, lack of well-being and a number of other symptoms. Dr. Whitham has put together several
